Forefront Living is the not-for-profit, faith-based parent of Presbyterian Village North, Faith Presbyterian Hospice, and the T. Boone Pickens Hospice and Palliative Care Center.ESSENTIAL DUTIES:
\tModel a service attitude towards others, is friendly and courteous to residents, patients, visitors, and co-workers
\tTake ownership in solving problems and take the initiative to make things better
\tEnsure resident, patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
\tMaintain good attendance record
\tDocument all assessments, reassessments, progress notes, discharge summaries and other clinical work in the medical record per departmental policy and procedure
\tProvide resident, family and staff education pertaining to Speech Therapy modalities to ensure understanding and proficiency of care
\tDemonstrate good clinical judgment in all aspects of resident treatments in which trained to provide
\tOrient, instruct, and direct work activities of Clinical Fellows and Speech Therapy students.
\tSupervise and review documentation written by the Clinical Fellow as per state practice act
\tScreen residents to determine their potential benefit from Speech Therapy intervention
\tDevelop, direct and/or participate in Speech Therapy programming for the residents
\tAttend in-services and other meetings as required on an ongoing basis
\tMaintain continuing education per state licensure requirements
\tOther duties as assigned by the Director of Rehab
MINIMUM QUALIFICATIONS:
\tLicensed Speech-Language Pathologist in the current state
\tMasters degree in Speech Language Pathology with Clinical Certificate of Competency or in Clinical Fellowship Year
\tKnowledge of therapy industry and federal, state, and local government regulations
\tDemonstrated ability to organize, prioritize and complete multiple tasks in a dynamic environment
\tDemonstrated efficiency strategies for fast paced environment
\tExperience in SNF/LTC/IL/AL/OP setting or mature population rehabilitation and wellness needs
\tExperience with PDPM treatment and reimbursement policy
\tBasic computer skills, Outlook, Word, Excel
\tExcellent customer service skills
\tGenuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients
\tAbility to perform work tasks within the Physical Demand Requirements
\tAbility to perform Essential Duties
\tAbility to comply with the Resident Bill of Rights and the Employee Responsibilities
\tAbility to comply with organizational and departmental safety policies and procedures
