    *HealthMark Rehabilitation *is an _inhouse_ therapy company, serving the Skilled Nursing Facility patients and residents of *HMG* facilities.

    We're currently seeking a *Full-time & PRN Physical Therapists, PTA's, Occupational Therapists, COTA's, and Speech Language Pathologists * to evaluate and treat residents and short-term patients at our *HMG Healthcare* facilities in *DFW and surrounding areas of Texas*.

    _Crowley Nursing & Rehab *(OT-PRN, PT-Full-time & PRN, PTA-PRN, SLP-PRN)

    • Harbor Lakes-Granbury (COTA-Full-time, OTR-PRN, PT-Full-time & PRN, PTA- Full-time & PRN, and SLP-PRN)
    • Holland Lake - Weatherford (PT-Full-time)
    • Lonestar - Stephenville (COTA-PRN, OT-PRN, PTA-PRN)
    • Methodist Transitional Care-Desoto (PT-PRN WEEKENDS, OT-PRN WEEKENDS)
    • Pecan Bayou- Brownwood (COTA-PRN, PT-PRN, SLP-PRN)
    • Red Oak (PTA-PRN)
    • Silver Springs- Abilene (COTA-PRN, OT-PRN, PT Full-time $5,000 SOB & PRN, PTA-PRN)
    • Treviso -Longview (PT-Full-time $5,000 SOB & OT $5,000 Sign on bonus)
    *Provides therapy services in accordance with physician's orders and accepted professional practices.

    • Registered therapists, develop an overall therapy plan of care to include: treatment and medical diagnosis warranting skilled intervention, planned duration and frequency to be provided, short and long term goals functional and measurable that support the discharge plan; Documents observations, patient's response to skilled interventions/treatment programs, degree of progress and notable changes in patient's condition within specific timeframes.
    • Completes all documentation required timely and in accordance with state and federal regulations and policies/guidelines.
    • Completes all billing required on the date of the provided skilled services, and in accordance with all state and federal regulations, policies/guidelines.
    • Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
    *PT/PTA/CCC-SLP/OT/COTA.

    • Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
    • Experience in a geriatric rehabilitation setting preferred.
