- Conduct nursing assessments both on-site and in the community to provide a written summary of health conditions and problems requiring medication care or consultation
- Observe assigned participants, collect vitals, record significant conditions and reactions, and notify supervisor or physician of participant's condition and reaction to drugs, treatments, and significant incidents
- Complete all documentation of services in an accurate, complete, and timely manner and follows agency, program, and Medicaid Rule 140 requirements
- Nursing degree from accredited university/college required
- Valid Illinois RN license required
- 1+ years of experience working with persons with psychiatric disorders and dual-diagnosis required
- Valid driver's license required
