Job Description:
Case Manager for Clients Attached to Behavioral Health - Work with existing case managers to exchange resources and collaboration

Considerations:
A certified Case Manager certificate or degree will be given extra consideration as will experience in the field of homeless services and/or Behavioral Health


Primary Responsibilities:


  • Enhance the quality of client management during their stay at Harvard and create a case plan to include opportunities and connections to minimize their time at Harvard, supporting their transition to more stable and supportive housing.
  • Work with clients who are experiencing various challenges in the area of Behavioral Health
  • Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client's health and human services needed.
  • Collaborate as needed with the social worker assigned to the client.

Responsibilities

  • Coordinate and provide client care that is safe, timely, effective, efficient, equitable, and client
- centered


  • Handle hard-case resident assignments, draft service plans, review case progress and determine case closure in the Homeless Management Information System (HMIS).
  • Working knowledge of HMIS required
  • Motivate clients to achieve confidence, success and autonomy where possible
  • Be familiar with work readiness programs, GED educational systems, obtain personal documentation such as Social Security cards, birth certificates, ID cards, immigration services
  • Connect with any and all service providers to promote successful outcomes
  • Be familiar with opportunities for community service hours
  • Be an advocate in homeless court
  • Be prepared to work with probation, homeless court, behavioral health, continuum of care, One
Stop, hospital case managers, Child Protective Services, Adult Protective Services, social workers and advocates


  • Facilitate multiple care aspects (case coordination, information sharing, etc)
  • Help clients make informed decisions by acting as their advocate regarding their participation in various programs
  • Develop effective working relations and cooperate with other service providers throughout the entire case management process
  • Arrange abeyances with social services, law enforcement, health and governmental agencies
  • Take the extra mile and interact with residents to keep track of their progress and ensure satisfaction
  • Record case information, accurately complete all necessary forms and produce statistical reports
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psycho-social issues
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Meet regularly with Shelter Manager and Executive Director to confer on progress and outcomes
  • Attend Weekly Staff Meetings Tuesdays from 1 pm to 2:30 pm

Requirements:


  • Proven working experience in case management, including as a Nurse, Medical, Mental Health
Care Manager or a related job


  • Excellent knowledge of case management principles
  • Previous experience with psychological aspects of care or equivalent
  • Effective communication skills
  • Effective organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate, trauma informed care - approach to all tasks and complimented by teamwork skills
  • Current license, certification or registration will be given extra consideration
  • BS degree in related healthcare field will be given extra consideration

Learned Requirements:

Working knowledge of the buildings safety systems
Working knowledge of the buildings camera system
Working knowledge of the mission and goals of SPIRIT of Santa Paula


Requirements:

Must have negative COVID-19 test
Must have negative TB test
Have a valid California Driver's License


Pay:
$ $25.00 per hour


Expected hours:
No more than 40 per week
