Nursing Leadership opportunities available in an integrative, team-based model of patient care
Our Nursing team leads are responsible for supervising the clinic operations and the coordination of the workflow of medical services for a primary care team in delivering the highest quality patient care
Responsibilities will include:
- Providing leadership to nurses, medical assistants, and clinical support staff to accomplish organizational and team goals for patient care, quality, patient and staff satisfaction.
- Conducting comprehensive patient assessments, managing plans of care for the patient/family/care team.
- Assisting team in provision and coordination of patient care, including case management/care coordination and follow up, panel coordination and management, medication reconciliation, barrier assessments and patient and family education.
- Participating in QI activities with team and QI committee to consistently improve outcomes and patient health.
- Promoting a continuous learning environment for new and existing staff.
Bachelor of Science in Nursing, with 3 years+ experience in a supervisory role or demonstrated leadership skills/ training.
