JOB SUMMARY:
Age Well's Care Management team provides care to South Orange County adults over the age of 60 who need assistance and direction in their abilities to live independently at home in a safe environment.
This position requires driving to local clients' homes for assessments (mileage reimbursement provided) and assisting with general Care Management duties.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Well versed in social services and resources available to seniors in Orange County.
- Assists with quarterly reassessments to get updates on how clients are doing and create a care plan as to what is currently needed (internal resources and external community partnerships/entities).
- Completes inperson annual reassessments (home visits) to determine current needs (develop new care plans with client).
- Ability to complete various end of month reports and provide supportive documents to data team.
- Provide assistance/resources to seniors (in office, over the phone) when requesting help and care management support.
- Assists with identifying, multitasking, and managing multiple needs of a caseload (effective time management with caseload that may increase in numbers).
- Excellent case note taking, and excellent communication with peers.
- Ability to identify if a senior's needs are out of scope of Age Well's ability to assist; and then identify with prospective senior other more suitable programs of support (i.e. APS, SHOPP, placement to higher level of care, etc.).
- Warm personality and demeanor and good interpersonal skills with people of all types of backgrounds, race/ethnicities, ages, etc.).
- Other duties as assigned by the supervisor.
EDUCATION & EXPERIENCE
- Education in Gerontology or related Human Services field ideal.
- Minimum 1 year of experience in the field of aging or equivalent combination of education and experience.
- Knowledge of resources available in Orange County.
- Knowledge of human services available to seniors.
- Must be computer literate and demonstrate proficiency in MS Word, Excel, Outlook, Salesforce.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, procedure manuals and reports; ability to communicate effectively. Bilingual Spanish a plus.
CERTIFICATES, LICENSES, REGISTRATIONS
- Valid California Driver's License.
- Valid Vehicle Insurance.
- Valid Vehicle Registration.
