- Demonstrates a deep understanding of the psychosocial effects of cancer on patients and families at various stages of the cancer journey.
- Addresses practical concerns such as financial difficulties, changes in relationships, custody planning, caregiver responsibilities, advance care planning, and psychological and spiritual distress.
- Works closely with the medical team to ensure the needs of patients and families are met. Participates in Value Based Care initiatives including distress screening, advance care planning, and survivorship support.
- Assists in the transition to Survivorship and addresses associated issues.
- Conducts psychosocial assessments and provides follow-up care, considering social support and community resources.
- Organizes support groups in collaboration with local agencies to improve patient care.
- Educates clinic staff on the role of a masters-prepared social worker, serves as a resource in managing patients' psychological needs, and addresses issues such as staff well-being.
- Provides information on community resources to help patients access diverse services.
- Builds relationships with community agencies and healthcare providers to enhance patient support.
- Advocates for patients and families within the healthcare system, facilitating communication with the medical team.
- Keeps detailed and timely records of all interventions in the EMR system.
- Delivers presentations to raise awareness of the psychosocial aspects of the cancer experience.
- Participates in quality improvement activities and may oversee staff coordinating patient services.
- Supervises MSW Student Interns, providing guidance and training.
- Demonstrates understanding of age-related differences in patient care.

Provides counseling and supportive services to patients in need of emotional and psychosocial assistance. Offers information on organizations providing emotional, physical, social, and financial support to patients and their families. Ensures compliance with the US Oncology Compliance Program, including the Code of Ethics and Business Standards.
**Responsibilities:**
**Qualifications:**
Requires a Master's degree in Social Work from a CSWE-accredited program. Ideally, three to five years of experience as a masters-prepared social worker in a healthcare setting, with a preference for oncology, palliative care, or hospice experience. Must hold a valid state masters-level license or be working towards one.
**Physical Demands:**

**Work Environment:**
