Job Summary:
Under direct supervision, receives requests for filed documents, locates, pulls, delivers and re-files in accordance with established policies and procedures.
Essential Responsibilities:
Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws. Sorts and files medical records and documents in accordance with departmental policies and procedures.
Pulls medical records as requested by other departments and/or medical personnel; responds to telephone requests and STAT file request; documents, on appropriate form, required information for all records removed from department.
Delivers and retrieves medical records and deactivates charts as required. Files loose paperwork into medical records as received from other departments Pulls and combines previous medical records.Periodically checks incoming and outgoing files for accuracy and missing forms; locates missing documents and records; reconciles illegible or missing data; reactivates inactive files.
Photocopies a variety of materials. Returns completed materials to requestors by mailing or delivering items. Performs other routine functions to include maintaining logs, answering phones, opening and sorting and distributing mail, etc. Handles all documents confidentially. Demonstrates initiative by asking for more work, completing assignments consistently with good quality, volunteers for cross training. Establishes and maintains courteous and cooperative relations with the public and other staff.Prepares folders for new patients by sorting and inserting forms, reports, and tests and inventories new charts for data processing.
Repairs charts and documents by replacing folders and/or metal fasteners and chart thinning. Provides patient information to authorized medical or administrative personnel. Sorts lab results and delivers to appropriate physicians. Prepares copies of appointment lists and distributes copies, prepares chart requests form lists and assembles in numerical order. Performs routine maintenance on office equipment Ability to identify nonstandard forms and determine action required. Utilizes various office equipment including, but not limited to, fax machines, multi line phones, PCs, photocopiers, printers, scanners, etc. Performs other duties as assigned by supervisor.Basic Qualifications:
Experience
Typing 20 WPM Demonstrated ability to perform the duties and responsibilities of the job at a fully satisfactory level as specified by established department performance standards.
