• Schedule: PRN Monday - Friday, at least one shift per week 10 hour shifts

Coordinate the interdisciplinary plan of care for patients from admission to discharge

• Create care plans, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations, and patient records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialogue with supervisor and health providers for effective health plan implementation.
• Act as a resource for medical record documentation, service appropriateness, and treatment options.

Licensure/Certification/Registration:

• Registered Nurse credential from the Alabama Board of Nursing required.
• BLS Provider preferred (AHA or ARC accepted).

Education:

• Diploma from an accredited nursing school/college OR professional licensure at hire.
