    Job Title: Inpatient Care Manager (RN or LSW) at MaineGeneral

    Investigates, assesses, and plans interventions to assist patients in dealing with social, emotional, economic, and environmental issues.

    Position Overview:

  • This per diem role offers schedule flexibility.
  • What You Will Be Responsible For:

  • Conduct interviews and assessments of patients and their families, caregivers, or legal representatives. Determine, prioritize, provide, or coordinate needed internal and external services.
  • Collaborate with multidisciplinary teams to assess patient status and the appropriateness of medical care, ensuring care coordination across all facilities.
  • Engage in discharge planning, identifying and coordinating suitable resources, providing education, and referrals as needed.
  • Review charts to identify service delivery issues, patient outcomes, compliance, and reimbursement concerns.
  • Your Qualifications:

  • Must hold Maine state licensure as a Licensed Social Worker (LSW) or Registered Nurse (RN)
  • Strong teamwork, communication, and critical thinking skills
