- This per diem role offers schedule flexibility.
- Conduct interviews and assessments of patients and their families, caregivers, or legal representatives. Determine, prioritize, provide, or coordinate needed internal and external services.
- Collaborate with multidisciplinary teams to assess patient status and the appropriateness of medical care, ensuring care coordination across all facilities.
- Engage in discharge planning, identifying and coordinating suitable resources, providing education, and referrals as needed.
- Review charts to identify service delivery issues, patient outcomes, compliance, and reimbursement concerns.
- Must hold Maine state licensure as a Licensed Social Worker (LSW) or Registered Nurse (RN)
