ESSENTIAL DUTIES & RESPONSIBILITIES

  • Schedule and complete initial intake assessments and all required screeners for clients.
  • Assist clients in accessing housing resources and adhering to medical care, medications, behavioral health services, oral health care, medical nutritional services, entitlement and/or benefit programs and other social services as needed.
  • Provide healthcare support to members (e.g., 340B patients) as necessary, including but not limited to healthcare provider visit followups, HIV and/or STI/STD education, and healthcare navigation assistance and services.
  • Develop comprehensive individual treatment plans for each client consisting of measurable goals and objectives. Monitor, update and reevaluate individual treatment plans at least once every six months.
  • Provide housing counseling services, assist with unit location and inspections, and process client requests for housing related financial assistance.
  • Cultivate, interface, and maintain relationships with housing partner agencies, prospect landlords and current owners of rental units.
  • Maintain client files in accordance with standards of care, funder contracts and agency policies.
  • Provide crisis intervention and/or psychosocial counseling to clients as necessary.
  • Document all client encounters in agency databases and the electronic health record system in accordance with timely documentation policies.
  • Facilitate support groups as assigned.
  • Ensure compliance with HIPAA requirements, state and federal policies governing social services, and the NASW Code of Ethics.

QUALIFICATIONS

SALARY AND BENEFITS

The compensation package for this position includes:

  • Starting Range of $48,000 to $51,000;
  • Generous Paid Time Off (PTO);
