ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provide rapid HIV testing and other prevention and care services in accordance with agency values, guiding principles, policies, and procedures.
  • Develop comprehensive individual treatment plans for each member consisting of measurable goals and objectives. Monitor, update and reevaluate individual treatment plans at least once every six months.
  • Facilitate support groups as assigned.
  • Provide healthcare support to members (e.g., 340B patients) as necessary, including but not limited to healthcare provider visit followups, HIV and/or STI/STD education, and healthcare navigation assistance and services.
  • Coordinate and provide HIV education and health promotion services, community outreach, HIV testing on
- and off-site, and linkages to care as appropriate.

  • Engage individuals in harm reduction strategies including PrEP/PEP, condom distribution, and sexual health education.
  • Provide crisis intervention and referrals as necessary.
  • Coordinate transportation needs and provide transportation as requested.
  • Provide ongoing supervision to assigned interns.
  • Input units of service into agency databases weekly.
  • Utilize the agency's electronic health record system to document all encounters in accordance with timely documentation policies.
  • Maintain accurate files for compliance with standards of care, funder contracts, and agency policies.
  • Compile required reporting documents as assigned for funder reports and monitoring of service provision.
  • Ensure all services are compliant with HIPAA requirements, state and federal policies governing such care, the NASW Code of Ethics, and agency values.
  • Identify service gaps and recommend policies and procedures that will contribute to a safe and effective prevention program.

QUALIFICATIONS
You might be a great fit for this position if you:

  • Are a certified HIV Tester in NJ (on-the-job training will be required to become a certified HIV tester if not already certified);
  • Have at least one year of applicable professional and/or internship experience;
  • Hold a Bachelor's degree in Social Work or related field;
  • Are committed to social justice initiatives for people living with HIV+ and the LGBTQ+ community;
  • Treat others with compassion and empathy;
  • Have great time management and organizational skills;
  • Enjoy working in a teambased environment;
  • Feel passionately about EDGE NJ's mission.

SALARY AND BENEFITS

The compensation package for this position includes:

  • Salary range of $48,000 to $51,000 annually;
  • Generous Paid Time Off (PTO);
