Responsibilities:

Provide direct nursing care to residents under the supervision of a Registered Nurse (RN)
Administer medications and treatments as prescribed
Monitor residents' condition and report any changes to the nursing supervisor
Assist with activities of daily living, including bathing, grooming, and mobility
Collaborate with interdisciplinary team members to develop and implement individualized care plans
Maintain accurate and uptodate medical records and documentation

Qualifications:

Current LVN license in the state of (insert state)
CPR certification
Prior experience in longterm care or skilled nursing preferred but not required
Excellent communication and interpersonal skills
Ability to work effectively as part of a team
Compassionate and patientcentered approach to care
