To reduce the length of homelessness, providers shall house 80% of program participants within 45 days of completing intake and all program participants within 90 days of completing intake.

2.

At least 80% of exiting individuals will be in permanent housing at closure, whether in a unit they were placed in or an alternative permanent housing solution.

3.

At least 85% of individuals that exit rapid rehousing to permanent housing should not return to homelessness within one year of program exit.

MAJOR DUTIES AND RESPONSIBILITES '¢ Facilitate mediation with participants and case managers '¢ Develop tracking systems to allow for accurate reporting '¢ Identify training needs and assist with training for staff '¢ Participate in Providers meeting '¢ Collaborate with the DHS/RRH-I Team to implement strategies including the development of inter and intra-agency relationships that support the continuum of services to Participants.

'¢ Acts as a liaison with landlords and other Community Organization to build relationships and to promote the success of RRH-I Program.

'¢ Ensure that the Agency is following all District policies and procedures related to homeless service programming '¢ Coordinate and conduct intake meetings/trainings with new program staff to explain the RRH-I Program.

'¢ Liaison and coordination of support between the case managers and the fiscal/rental assistance staff to ensure timely tracking and projections of financial assistance packages.

'¢ Reviews DHS identified database and HMIS for data quality and completeness. '¢ Conducts monitoring of program performance on a regular basis.

MINOR DUTIES AND RESPONSIBILITIES Manager the duties and responsibilities of the Case Manager Supervisor, Case Management Team, Housing Navigator, and Employment Specialist.

Communicate Program responsibilities, timelines, outcomes, and deadlines. Maintain appropriate training and staff development responsibilities. Communicate and attend meetings with agency and community stakeholders.


QUALIFICATIONS FOR THE JOB Education:

The Program Manager must have at minimum a master's degree in the Human Services, Health Care, or Social Work field, or a Bachelor of Science in Nursing or be Licensed Clinical Social worker and meet the additional requirements listed.

They must have professional knowledge of the theories, principles, techniques, and practices of social service delivery systems.

They must have a minimum of two years professional supervisory and/or managerial experience in the human services and/or social service delivery systems.

They must have a minimum of four years of professional homeless service experience. KEY COMPETENCIES Ability to manage agency and contractual requirements Maintain effective communication both orally and in writing Ability to motivate and sustain a others during crisis and changes within the agency and contractual responsibilities Ability to cultivate and sustain a professional work environment Ability to maintain fidelity to the Scope of Work and the contractual obligations Ability to meet with Contract Administrators and related personal at the Department of Human Services
