Desert Mountain Care Center (formerly Indio Nursing And Rehabilitation Center ) is a state-of-the-art skilled nursing and subacute facility featuring 99 beds located in
Indio,
CA.

We are looking to fill in the director of social services position for our facility.

Job Duties:

Develop and implement policies and procedures for the identification of medically related social and emotional needs of the residents.
Participate in community planning related to the interest of the facility and the services and needs of the residents and family.
Participate in discharge planning; development and implementation of social care plans and resident programs.
Interview resident/families as necessary and in a private setting. Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Administrator as required.
Involve the resident/family in planning social service programs when possible. Assist in arranging transportation to other facilities when necessary. Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.

Requirements:

Must have SSD Certification
Must have, as a minimum, a bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
Must have, as a minimum, 5 years of experience as a Social Worker, preferably in a hospital, longterm care facility, or other related health care facility.

Pay:
$32.00 per hour

Expected hours: 40 per week
