    Responsibilities:

    • Verify the plan of care with the RN before each visit
    • Collaborate with physicians and RNs to meet patient needs
    • Educate patients and caregivers on self-care
    • Monitor vital signs and medications
    • Perform specialized procedures as ordered
    • Prepare equipment for treatments
    • Complete documentation accurately

    Qualifications:

    • 1 year of clinical LPN experience
    • Current LPN license
    • CPR certification
    • Valid driver's license and reliable transportation
