Position Overview


A Community Health Worker is a front-line public health worker who is a trusted member of and/ or has unusually close understanding of the community served.

This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.


A CHW also builds individual and community capacity by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.


Location and Hours of Position
PHN Schuylkill Community Health Center. 210 Sunbury St. Minersville, PA 17954

The general office hours for this position are 8:30 AM-5:00 PM.

Traveling to our Shenandoah, and Shamokin offices will be required as needed.


NO WEEKENDS

Qualifications

  • Bachelor's Degree
  • Certified Community Health Worker in PA or be willing to work towards it.
  • Possession of a valid driver's license.

Duties

  • Cultural Mediation among individuals, communities and Primary Care Home.
  • Provide culturally appropriate health education/information.
  • Care Coordination, Case Management and system navigation.
  • Provide coaching and social support.
  • Advocates for individuals and communities
  • Build individual and community capacity.
  • Provide Direct Service.
  • Implements individual and community assessment.
  • Conducts outreach.
  • Participates in evaluation and research.
  • Explores each person's specific barriers to receiving care and develops and implements an individual plan to address barriers.
  • Meets with patients following each appointment to help direct them to the laboratory or to other appointments.
  • Provides emotional support, validates patients' feelings, asks openended questions, and listens reflectively.
  • Leads and assists with group peer support meetings focused on coping with life with chronic disease, stress management, group empowerment, and other groupselected activities.
  • Interviews individuals about their health concerns, including survival and social concerns such as parental stress, nutrition, access to medical care, crime, domestic violence, mental health, and substance abuse.
  • Provides links to supportive community resources and tracks referrals made to local programs to address patientidentified community and policy issues affecting disease management.
  • Provides limited medication reconciliation without making recommendations Counsels patients on medication adherence, uses physicianapproved protocols.
  • Assesses patients needs and provides personalized assistance.
  • Explores each patients specific barriers to receiving care and develops and implements an individualized plan to address these barriers, such as scheduling appointments, resolving insurance, accompanying patients to followup appointments, and making home.
  • Identifies, trouble shoots, and responds to patients concerns, such as reminders and transportation and assistance for upcoming appointments.
  • Makes weekly telephone calls to patients to discuss overall wellbeing, adherence to action plans.
  • Clarifies questions stemming from patients encounters with health care providers, acts as an interpreter to enhance communication between patients and providers, reinforces teaching provided by health care providers. Assists patients in reading medical forms to address limited functional.
