    Description

    The LEAD Supervisor is responsible for managing day-to-day program operations and providing responsive supervision to all LEAD employees. This position supervises employees based primarily in a street-based outreach context.

    The LEAD (Let Everyone Advance with Dignity) Program Supervisor is responsible for the day-to-day supervision and coordination of the LEAD team. The LEAD Program Supervisor oversees a multi-disciplinary team that provides comprehensive street outreach and ongoing navigation supports individual goal planning, intensive case management, peer services, housing, and care coordination. The LEAD team coordinates with community resources to provide access to address varying client needs, including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing affordable housing, accessing employment and educational opportunities, and accessing supports for basic needs.

    This position requires flexibility of hours, independent decision-making, program planning, budgetary compliance, staff supervision, quality management, and being a liaison with numerous outside agencies.The Program Supervisor is directly responsible for the supervision of the LEAD team members (including peer specialists, admin assistance, screening and outreach coordinator, and all Case Managers).

    Schedule: Monday- Friday, 8:00am - 4:30pm

    Minimum Requirements:

    • CADC II or above required.
    • QMHA required
    • Must be registered with appropriate Oregon State licensing board to meet, at minimum, QMHA requirements.
    • One year relevant and progressive experience required to include lead or supervisory experience.
    • Preferred; Master's degree in a human services field and ability to register with appropriate Oregon State licensing board to meet, QMHP.
    • Experience working with outreach teams preferred.
    • Must have current CPR certification prior to start.
    • Must embrace the mission and philosophy of harm reduction, and client centered outreach supports.
    • Must meet CCC privileging requirements as required by FTCA.
    • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
    • Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy.Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
    • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
    • Will be required to carry an agency cell phone for work use.Cell phone will be provided by Central City Concern.
    • Required to be available for after-hours pager and holiday coverage rotation.
    • Must adhere to agency's non-discrimination policies.
    • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.Able to climb stairs several times a day.
    • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliations, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
    Responsibilities and Duties:
    • Ensure that LEAD program services are practiced according to the highest professional and ethical standards and assure that the health and safety needs of clients are met.
    • Hire, coach, manage and supervise all LEAD staff members.
    • Oversee daily operations for the team, including team huddles, co-outreach needs, coverage needs and staff support.
    • Coordinate with the Screening and Outreach Coordinator and Case managers to ensure that daily activities for enrolled clients are planned, implemented, and coordinated with the team and that referrals are followed through in a timely manner.
    • Quickly and accurately support team members in establishing client crisis stabilization in a community setting.
    • In collaboration with the Director of Outreach, develop and maintain systems to ensure compliance with regulations, licensures, credentials, and contractual stipulations, including oversight and management of the patient records system.
    • Ensure that CCC policies, procedures and expected practices are implemented, maintained, and followed.
    • Provide and document at least 2 hours of supervision per month for each full-time staff supervised.The two hours must include one hour of individual face-to-face contact for each person supervised and a minimum of one hour of group supervision.
    • Ensure clinical privileging requirements are met for all direct reports pursuant to CCC's Privileging policy.
    • Review documentation to ensure that services delivered are clinically appropriate and effective. Complete annual performance reviews, implement corrective action plans when indicated.
    • Participate in collaborative Operational Team meetings with partner agencies (such as Clackamas County Sheriff's Office, Clackamas County District Attorney's Office, Clackamas County Health, Housing, and Human Services liaisons, courts).
    • Review and appropriately identify level of follow-up needed with critical incident reports. Facilitate critical incident debriefings.
    • Process client grievances.
    • Represent Central City Concern LEAD program in the greater community.
    • Perform direct services to clients, including case management, assessments, and outreach activities as needed.
    • Identify and coordinate staff training needs and ensure the team remains committed to a harm reduction model.
    • Work cooperatively with Senior Director of Substance Use Disorder Services, Director of Outreach Services , Screening and Outreach Coordinator, CCC compliance officer and Quality Manager to develop and maintain QI/UM plan and outcome data collection.
    • Develop and implement policies with assistance from and Director of Outreach Services.
    • Maintain appropriate clinical certification(s).
    • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access.This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
    • Perform other duties as assigned.
    Skills and Abilities:
    • Knowledge, education, training, skills, and experience in providing and supervising services in a highly diverse work environment to a highly diverse and mostly homeless population.
    • Knowledge, education, training, skills, and supervisory experience in an integrated clinical care model.
    • Demonstrated knowledge of Motivational Interviewing, Stages of Change, and Harm Reduction. Demonstrated knowledge of Trauma-Informed Care and Anti-Racist clinical practice.
    • Demonstrated knowledge and skills in counseling techniques, to include mental health and substance use disorder treatment methods.
    • Ability to maintain accurate records and necessary paperwork. Must be able to follow screening, enrollment, assessment, and client goal planning protocol.
    • Knowledge of budgeting practices and procedures.
    • Ability to manage time and meet deadlines.
    • Ability to display a non-judgmental attitude.
    • Ability to communicate clearly and concisely, both orally and in writing with clients, co-workers, and community partners.
    • Ability to work courteously and effectively communicate with the public, medical personnel, corrections personnel, police, and coworkers.
    • Ability to work independently, plan, organize, prioritize, implement, and multi-task.
    • Must be creative, innovative, and be a progressive problem solver.
    • Ability to attend to detail, be flexible and dependable.
    • Must be able to work under stressful conditions.Must be able to work in an environment where people may be verbally hostile.
    • Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
    • Have the necessary computer skills to accomplish duties.
    • Must be able to work variable work hours.
    • Sufficient manual dexterity and physical ability to perform assigned tasks.
    • Physical ability to bend, stoop, kneel, squat, twist, reach, pull, lift heavy objects, and climb stairs several times a day.
    • Must be able to perform considerable intra-city travel.
