    WHAT YOU WILL DO
:


The Case Manager position will provide direct services to clients, to include but not limited to, intake, assessment and case management, and client advocacy (resources and referrals) to homeless and low-income individuals, with the goal of moving clients from crisis to self-sufficiency and to support the mission of Interfaith Community Services.


  • Provides direct services to clients including diversion, Individual Service Plan (ISP), assessment, crisis intervention, advocacy, social services, case management and vocational case management
  • Assists participants in development of a strengthbased/solutionfocused individualized goal and action plan that promotes permanent housing and selfsufficiency; develop an effective, timely referral network of services and support that assist with housing retention in order to ensure ongoing direction and support as needed
  • Coordinate and provide services that are comprehensive, timely, effective, efficient, equitable, and clientcentered
  • Identifies resources and advocates for clients' needs
  • Links clients with resources within the agency and utilizes other public and private social service agencies and services as necessary, such as CalFresh, WIC, Social Security, and State Disability to increase household income
  • Develops financial goals with the client including developing and maintaining a household budget, becoming banked, paying rent and other bills on time, and increasing household income and credit score
  • Assists in development of and encourage adherence to a personal budget through proactive housing and budget counseling sessions; provide budget counseling and education to assist clients in establishing payment plans for bills and past debts and to assist clients in obtaining and maintaining their housing and selfsufficiency
  • Ability to work independently and professionally at partnering agencies, faith centers and other community settings
  • Ability to work with and collaborate with volunteers, interns and other agency personnel
  • Accurately record case information within electronic databases (HMIS/Clarity, CIE, ETO, and Excel) and client files, accurately complete all necessary forms, produces statistical reports and utilizes the Coordinated Entry System
  • Participates in all staff meetings and trainings
  • Responsible for meeting specific outcomes required by program contracts
  • Perform additional duties as assigned

Requirements:


EDUCATION/ EXPERIENCE:


  • HS Diploma or equivalent and 1+ year experience required
  • Bachelor's Degree in Social Work or related field preferred or equivalent combination of education and experience preferred
  • Bilingual (English / Spanish) highly desirable
