Description
Responsibilities:
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient's condition.
- Evaluate patient needs in order to provide a personalized plan.
- Ensure patient's compliance with medication and care plans.
- Ensure the care our patients receive is coordinated, safe, timely, effective, efficient, equitable and patient centered.
- When needed, administer medications and treatments as prescribed by the physician.
- Maintain accurate and uptodate patient records, ensuring compliance with HIPAA regulations.
- Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans.
- Educate patients and their families about their medical conditions, treatment plans, and selfcare techniques.
- Other duties as assigned.
- Must be able and willing to travel between fascilities on a daily basis.
Experience:
- Minimum of 2 years of experience as a Medical Case Manager
- Minimum of 2 years as a Registered Nurse in an acute care setting
- Proficient in using electronic medical record systems such as Epic
- Experience in home care or community health settings is a plus
Skills:
- Acute pain management techniques
- Strong understanding of HIPAA regulations and maintaining patient confidentiality
- Excellent communication skills to effectively interact with patients, families, and healthcare team members
- Excellent followup and organizational skills
- Triage skills to prioritize patient needs based on severity of condition
