    Description

    Job Overview

    This position offers a variety of clinical social work services to patients and their families in different settings such as inpatient, ambulatory, and community, using individual, family, or group approaches while following professional social work ethics. Participates in multi-disciplinary case conferences. Collaborates with agency care providers to address social service needs of clients and families.

    Job Description

    Minimum Qualifications:

    • Master's degree in Social Work from an accredited institution
    • Licensed Clinical Social Worker (LCSW); NH licensure may be required based on territory and operational needs
    • One (1) year of relevant experience

    Preferred Qualifications:

    • Three (3) years of relevant experience
    • Bonus if Bilingual in Chinese
    • Experience in pediatrics is a plus

    Duties and Responsibilities:

    • Conducts psychosocial assessments of individuals and families, identifying limitations, deficits, and strengths, and develops appropriate treatment plans
    • Handles psychosocial assessments and interventions during crisis situations involving suicide, homicide, family violence, and abuse
    • Evaluates the need for long-term care, assists in developing care plans, and arranges placements if necessary
    • Provides therapy and counseling using individual, family, or group methods
    • Offers information, referrals, and resource acquisition for individual and family needs
    • Collaborates with healthcare teams to ensure comprehensive patient care
    • Assists in discharge planning and continuity of care
    • Engages in outreach and case finding for high-risk issues
    • Participates in policy and program development
    • Provides social services to patients' families
    • Ensures complete clinical documentation
    • Participates in professional education and supervision activities
    • Maintains collaborative relationships with colleagues
    • Monitors and modifies social work practice as needed
    • Ensures compliance with regulatory guidelines and agency policies

    Skills & Abilities:

    • Ability to provide care throughout the lifespan considering cultural patterns
    • Knowledge of casework process and community resources
    • Familiarity with social work theory and practice
    • Understanding of organizational theory and health care policies
    • Strong diagnostic, interpersonal, and communication skills
    • Proficiency in standard office programs
    • Excellent English communication skills
    • Outstanding interpersonal and organization skills
    • Capable of functioning well in busy environments
    • Responsible, reliable, and well-organized
