This role will encompass a wide variety of in-home skilled care needs such as creating care plans, evaluating service needs, assisting in the field with the in-home infusion team, and providing interviews/field staff training.

Office duties include assisting with orientation, following up with clients and supporting employee needs such as health assessments and PPD placements.

This position is a hybrid combining a mix of field and office duties.


Duties will include the following:


Create plans of care and treatment using sound judgment and critical thinkingProvide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process.

Perform wound care and recognize the need for and institute emergency measures in a rapid and appropriate manner.


Perform all skills, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adhere to the policies, procedures, and protocols of the agency when necessary.


Assist with assignment of staff by collaborating with other office teams for IV skilled nursing, Wound Care team, Private duty nursing team and Nonskilled care team.

Assist with employee onboarding requirements and facilitate the clinical portion of orientation as needed.

Requirements:

Current RN license in MA in good standing. Dual license in border state of NH preferred.

Certified CPR and negative TB skin test or chest x-ray

Minimum one (1) year of documented experience required in a home health agency setting.

IV placement and phlebotomy skills preferred.

Applicant must have a valid driver's license and active automobile insurance.

Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift to 50 pounds.

Compensation:
$85k-95K salary depending on experience with additional Vehicle allowance.
