    Island Nursing Rehab Center -

    We are a 120-bed not-for-profit nursing facility offering advanced healthcare while providing skilled nursing and rehabilitative services for both long and short term residents. Located on eight beautiful landscaped acres, we also provide sub-acute services, wound care and respite services.

    The Vision of Island Nursing and Rehab Center is to achieve the highest level of resident/patient care services by focusing on resident/patient satisfaction and to serve as a center of excellence for our specialty programs, as well as a high-quality provider of care to the elderly and physically frail population in our community.


    GENERAL OBJECTIVES:


    Employee assumes responsibility in adhering to planned, organized, implemented evaluating programs of PPS assessments and OBRA regulations. The omission of specific statements of duties does not exclude them from the position if the work is similar, related, or is a logical assignment to the position and your licensure.


    QUALIFICATIONS:


    A current New York State licensed Registered Professional Nurse who possesses a Registered Professional Nursing degree from an accredited college school of nursing program. Bachelor's degree preferred. Two years of clinical nursing experience and one year geriatric experience strongly preferred. Demonstrated leadership ability in nursing practice in a Long Term Care setting required. Must possess and maintain all necessary certifications for CPR/BLS and other certifications, in-services as required by this facility. Completion of the MDS 3.0 training experience required. Employee must possess a working knowledge of the Medicare/OBRA assessment and regulations in a Long Term Care Facility. Employee must possess working knowledge of computer and associated skills.


    SPECIFIC RESPONSIBILITIES:


    1. Insures all Medicare determinations are completed in a timely manner.


    2. Assesses that all necessary documentation required for Medicare coverage is completed within required timeframes.


    3. Responsible for completion and accuracy of certification forms, re-certification forms and end of skilled care forms.


    4. Delegates' responsibilities for completing sections of the MDS 3.0 to staff that have clinical knowledge about the resident.


    5. Participate in the Interdisciplinary Care Plan meeting to assess resident needs and insure that the activities of the committee result in proper identification of problems, complications and risk factors with regard to residents/patients.


    6. Participates in departmental and interdisciplinary committees, which influence or determine policies affecting nursing practices.


    7. Represents INRC as a Case Manager to establish an on-going professional relationship with the affiliated hospital, and health insurance partners.


    8. Provide required clinical updated information to support the need for Service at INRC, to outside Case Managers.


    9. Creates and maintains an atmosphere of courtesy and respect toward employees, residents and families in an enthusiastic manner.


    10. Maintains professional competence through participation in continuing education and other appropriate learning experience.


    11. Reviews/signs off on MDS 3.0 as mandated by federal regulations for RN signature when completed for accuracy and completion.


    12. Processes information from staff to make appropriate RN assessment and determination if a significant change assessment is warranted.


    13. Performs other duties as assigned by MDS Coordinators and/or Director of Nursing Services or designee.


    14. Demonstrates knowledge, respect and sensitivity of resident's rights as identified within the Resident Bill of Rights and facility policy and mission statement. Assures confidentiality of all resident information.


    15. Attends in-service education as required by position title.


    Performs related duties as required.
