Duration : 1 year plus extensions

Job Summary:

Duties:

Conduct assessments to determine the needs and eligibility of individuals with disabilities
Develop individualized service plans based on assessment results
Provide counseling and support to individuals with disabilities, including crisis intervention and addiction counseling
Collaborate with community organizations and agencies to coordinate services for individuals with disabilities
Conduct regular followup meetings to monitor progress and make necessary adjustments to service plans
Provide mentoring and guidance to individuals with disabilities, promoting selfadvocacy and independence
Maintain accurate and uptodate documentation of all interactions and services provided

Requirements:

Bachelor's degree in behavioral health, social work, or related field (Master's degree preferred)
Experience in crisis management, addiction counseling, or related fields
Knowledge of behavioral health principles and practices
Familiarity with motivational interviewing techniques
Strong interpersonal skills and the ability to build rapport with individuals from diverse backgrounds
Excellent communication skills, both verbal and written
Ability to work independently as well as part of a team
Experience in supervising staff is a plus
