Case Manager:

Brighter Heights Maine is growing and we are seeking a behavioral health case manager to join our BHH team and work with children and adults in and around Portland.

This position is part time, with the ability for hours to increase. The first 90 days of employment requires working out of our Yarmouth office. Our case managers see clients in their homes or community with occasional telehealth visits.


Pay:
$32.50 per hour

Case Manager Qualifications:


  • MHRTC or provisional MHRT, required to get MHRTC in allotted time period
  • Bachelors degree required
  • At least 1 year of case management preferred, familiarity with supporting people with substance misuse is helpful

Case Manager Responsibilities:


  • Coordinate and provide care that is safe, timely, effective, efficient, and clientcentered
  • Handle case assignments, write treatment plans and progress notes, review case progress and determine case closure
  • Help clients achieve wellness and autonomy, meet with them regularly, support them in making informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • Link clients and families with local resources, agencies and providers
