    SUPERVISORY RESPONSIBILITIES
    None.


    SUMMARY
    Provides street-based outreach services to homeless individuals and families. Assists with outreach and identification of individuals, provide linkages to community resources and navigation services.


    Assists in the fulfillment of the agency's vision, "For Those We Serve," by empowering individuals who thrive as healthy, self-sufficient and contributing members of our communities.


    DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS
    Provides crisis intervention information to homeless individuals and/or their families.

    Participates in weekly coordinated street outreach with Home Team.

    Provides navigation services to clients identified through coordinated entry.

    Works with Coordinator and/or Program Manager to promote Sanctuary and Youth Services programs in the community.

    Conducts street-based outreach to locate high-risk population in targeted areas. Identify community resources and provide referrals to Sanctuary and Youth Services programs and/or other community organizations.

    Participates with local neighborhood associations or community-based organizations in the project areas.

    Maintains log of outreach activities and street contacts.

    Collects and documents data as required, and assists in compiling statistical data, Management Information Systems (HMIS), and monthly reports.

    Facilitates the intake and exit process of program participants, prepares and maintains all files in a confidential manner.

    Ensures that client files are complete and accurate as required by project guidelines (i.e. contains intake, assessment, individualized service plan, documentation of services provision/disposition, and referral outcomes).

    Provides full supportive services to clients including: client needs assessment, service plan, and regularly review and update goals.
    Provides information, referrals and follow-up for appropriate services and resources.

    Identifies common areas that runaway and homeless youth, and/or trafficked persons congregate. Assists in developing an outreach plan to identify homeless individuals in these high transiency areas.


    Collaborates with all Sanctuary and Youth Services programs, Fresno EOC programs, and partner agencies to ensure positive placement of clients.

    Maintains positive community relations with family members and/or other collaborative partners.

    Assists in daily program planning and leads recreational, social and academic activities.

    Attends staff meetings and trainings as required.

    Completes any child abuse referrals as needed, as required of all mandated reporters.


    Adheres to all guidelines, policies, procedures, and protocols of the Sanctuary and Youth Services Program and Fresno Economic Opportunities Commission.

    Provides transportation and advocacy for health, shelter, and social services. Pick-ups, delivers, and/or prepares and serves meals to clients.


    SANCTUARY OUTREACH TO THE STREETS:

    • Identifies and serves runaway and homeless youth under 20 years old.
    • Assist youth to identify appropriate housing and assist with referrals.
    • Provides supervision of clients accessing the Drop-in Center.
    • Logs and charts youth's behavior patterns, activities, and progress towards goals in service plan.
    • Facilitates workshops (e.g. independent living skills, life skills, etc.) and/or authorized activities with program participants.
    • Provides navigation services to youth identified through coordinated entry.

    CENTRAL VALLEY AGAINST HUMAN TRAFFICKING

    ONLY:


    • Identifies and connects victims of labor/sex trafficking or commercial sexual exploitation, (including minors and adults) to eligible services by working directly with Human Trafficking program staff. (Target populations may include runaway and homeless or at-risk youth, immigrants, and LGBTQ).
    • Conducts outreach, training and public awareness to potential victims, organizations who serve victims, survivors, law enforcement, and other stakeholders on the issue of Human Trafficking and reporting procedures in a six-county region including Fresno, Kern, Kings, Madera, Merced and Tulare counties.

    HOMELESS ENGAGEMENT RESOURCE OUTREACH (HERO)


    AND SUPPORTIVE SERVICES

    ONLY:


    • Conducts street-based outreach in Fresno and Madera Counties.
    • Assists City of Fresno Police Departments' Homeless Task Force with outreach, assessment, and/or transportation of homeless individuals or families.
    • Engages homeless individuals by conducting VI-SPDAT assessments, provides brief interventions and connects individuals to support services.
    • Provides navigation services to individuals identified through coordinated entry.
    • Attends all meetings related to outreach and/or coordinated entry. Any other meetings will be determined by the FMCOC Community Coordinator.
    • Reports on program participant challenges, successes and transitions during Annual Performance Reviews or other meetings as directed.

    TRANSITIONAL AGE YOUTH SHELTER (TAY SHELTER):

    • Identifies and serves runaway and homeless youth between 12-24 years old.
    • Provides referrals for emergency housing and transitional living.
    • Provides supervision of clients participating in the Drop-in Center and recreational activities on site.
    • Logs and charts youth's behavior patterns, activities, and progress towards goals in service plan.
    • Facilitates workshops (e.g. independent living skills, life skills, etc.) and/or authorized activities with program participantsmiR
    • Transports youth to health or housing appointments
    Travels for agency business using personal/agency vehicle.

    Maintains regular attendance and punctuality.

    Works evenings and weekends as required.

    Performs other duties as assigned.

    QUALIFICATIONS

    EDUCATION:
    High school diploma or GED.

    Minimum of 60 college units in a Human Services field required. Work experience may substitute required education on a year-by-year basis.


    EXPERIENCE:
    Minimum of one year related experience required.

    Prior experience working with youth, homeless populations or in a social service related area preferred.


    OTHER QUALIFICATIONS:
    Willing to work non-traditional hours and days to meet the needs of this position.

    Proficient in Microsoft Office Suite, related business software and web-based reporting systems.

    Effective presentation, written and verbal communication skills.

    Bilingual English/Spanish or other secondary language preferred.


    Basic knowledge of the Welfare and Institutions Code and other laws affecting children and families in the State of California preferred.

    Basic knowledge of Community Care Licensing, Title 22 Group Home standards for facilities, staffing, and services preferred.

    Familiarity and ability to work with youth/families of all cultural, ethnic, religious and socio-economic backgrounds.


    Pass required background investigation including medical examination and TB clearance, acceptable criminal record statement, Department of Justice fingerprint clearance, and child abuse index clearance.


    Maintain a valid California driver's license, reliable transportation, adequate auto insurance as required by State law, and insurability by agency carrier.

    CPR and First Aid training preferred; required within six (6) months of hire.

    It may be necessary to restrain a youth, or protect staff or other residents from physical attack.
