$62,000-$71,933

Founded in San Francisco in 1902, the LightHouse's purpose is to promote the independence, equality and self-reliance of people who are blind or visually impaired.

The LightHouse is currently in a period of growth, increasing our capacity to serve more low-vision and blind clients.

OVERVIEW:


Students will work with the Service Navigator to follow or amend their PATH plan at any time during their LightHouse service participation.

Students can be current or new to the LightHouse.

Under the guidance of the Director of Connections, the Service Navigator works closely with assigned students to ensure student's service goals are relevant and being met.


QUALIFICATIONS:


  • Minimum of a Bachelor's Degree in social work, rehabilitation, nursing, counseling, psychology, or related field
  • At least 2 years experience with client case management in a communitybased setting
  • Strong coaching skills, including positive problem solving and professional communication
  • Excellent communication skills both written and oral
  • Ability to clearly communicate concepts, processes and methods to enhance learning and skill building
  • Ability to read, analyze, and interpret professional health journals, technical procedures, and/or government healthcare and disability regulations.
  • Ability to write business correspondence, and effectively present information and respond to questions from groups of managers, students, customers, and the public.
  • Proficient computer user, particularly in Microsoft Office, Customer Relationship Management (CRM) and Electronic Health Record (EHR) systems
  • Maintain up to date First Aid and CPR certifications, to be paid for by LightHouse
  • Experience working with the blind/disabled community highly desired
  • Bilingual/ bicultural in Spanish highly desired

JOB RESPONSIBILITIES:


  • Collaborate with Intake Coordinator to ensure smooth transition from initial student intakes and assessments to LightHouse PATH plan
  • Collaborate with Intake Coordinator to ensure smooth handoff from Intake to Service Navigation and PATH plan
  • Work with department directors and program managers to ensure students have been contacted within ten business days of referral.
  • While students have an open case, checkin monthly with students to ensure PATH Plan meets student needs
  • In collaboration with Senior Director of Curriculum and Connections staff, develop structure to assure accurate and accessible training for students, including their goals and progress.
  • Maintain accurate and timely case notes on each student in Salesforce and Electronic Health Records (EHR) system
  • Work with LightHouse service providers to review and/or amend PATH Plan as necessary
  • Maintain active dialogue with students and their support networks, when appropriate
  • Assist students in signing up for external services and/or make external referrals
  • Collaborate with student and internal/external service providers to meet the student's PATH plan and goals to ensure student's success, or close services if warranted.
  • Be knowledgeable in all LightHouse services and promote service use and improvement across all of LightHouse's service offerings
  • Stay up to date in best practices of disability and aging services
  • Maintain confidentiality of sensitive information and follow LightHouse HIPAA guidelines
  • Ability to follow procedures established by external funding sources and medical service providers
  • When necessary, accompany students and their support networks to medical appointments, specifically related to vision care.

PHYSICAL REQUIREMENTS:


  • Ability to sit at a desk and perform computerintensive work for extended periods daily
  • Ability to operate standard office equipment
  • Must be able to independently travel to all LightHouse physical locations and events, as well as throughout the San Francisco Bay Area, to provide services as needed.
