Hope Hospice & Healthcare, Inc., a leading provider of end-of-life care, is seeking a dedicated professional full-time Hospice RN (Mon-Fri, 5d x 8h) to join our team.

The Hospice RN will manage a case load and assist with admissions as needed.

1 of 2 hospice agencies in all of Southern CA to be designated by CMS (Center of Medicare Services) as a 5-Star Quality Hospice Agency, and currently in our 4th consecutive quarter achieving this rank.

General Responsibilities include but are not limited to:

• Coordinates the services provided to his/her patients.
• Works cooperatively with the attending physician, hospice Medical Director, and other members of the interdisciplinary team.
• Participates in patient care conferences and supervises the hospice aides assigned.
• Regularly reviews and updates the plan of care.
• Performs prescribed medical treatments, including pain management and symptom control.
• Conducts assessments and evals, maintain excellent oral and written communication skills.
• Provides education and supportive care to patient and caregiver.
• Current CA RN License.

- Minimum of one-year experience as a professional hospice nurse within the last 3 years

• Valid driver's license, clean DMV record, auto insurance and dependable transportation required.
• BSN is preferred. Certification in Hospice and Palliative Nursing is desired.

Pay:
$ $50.00 per hour
