The work schedule is
Monday through Friday 8:30 a.m. to 5:00 p.m. (40 hours/week) with Sunday and Saturday off.


Duties and Responsibilities:
(these duties and responsibilities are not all inclusive)
:

  • Provide case management services for eligible youth under age 22, with majority of caseload below age 1
  • Position will be required to conduct comprehensive child centered family focused assessments, participate in service and treatment planning and
  • Develop disposition plans for youth that focuses on communitybased services.
  • Work with a team of internal and external providers and family to ensure integrated planning and service provision.
  • Ensure all documentation in clinical records and billing documents are completed.
  • Access community resources and monitor service provision with contracted providers. Youth will be involved with multiple systems such as school, outpatient treatment, DCF, DYS and MBHP services.
  • Monitors the implementation of Individual Service Plans (ISP)
  • Consults with service providers and community agencies on Child, Youth and Family Case Management issues; providing and/or arranging transportation of clients; and attends and participates in client service planning and discharge planning.
  • Plan, convene and chair meetings of client's care planning team to discuss ISP effectiveness and modifications, coordinate service delivery and other assistance.

Required Qualifications:


  • Knowledge of psychosocial problems, types and symptoms of those with mental and emotional disorders.
  • Knowledge of the principles, practices and techniques of counseling, interviewing, crisis intervention, motivation and reinforcement.
  • Knowledge of the types of programs and services available to the mentally ill population.
  • Ability to make case assessments by examining records and documents and through questioning and observing clients.
  • Ability to communicate effectively orally and in writing, giving clear accurate instructions.
  • Ability to make decisions, act quickly and maintain a calm manner in stressful or emergency situations.

Special Requirements:


  • Travel is required
  • This position directly manages cases in the DMH Quincy Site catchment areas and services in which will be primary travel locations; however, frequent travel is required throughout the DMH Southeast Area and occasional required statewide travel to various acute inpatient hospitals or other treatment settings. Current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license required.

Preferred Qualifications:


  • 3 years' experience in child/adolescent services.
  • Multilingual fluency.

Department of Mental Health provides Child, Youth, and Family Services to eligible individuals who meet the clinical criteria for service authorization.

In addition, we serve as the Mental Health Authority in the Southeast Area and collaborate, consult with and support implementation of mental health services in the Area.

Qualifications

  • Applicants must have (A) at least three (3) years of fulltime or equivalent parttime, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

Substitutions:

  • I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.
  • Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
  • Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
