Summary:

This position is responsible for directing and supervising the day-to-day operations of the social service department of their respective Life Plan Community


Essential Duties and Responsibilities:

Plans, develops, organizes, manages, and evaluates their location's Social Services department and staff ( if applicable)

Completes daily administrative paperwork including but not limited to budgeting, community planning, problem solving, assessments

Documents in residents' charts regarding pertinent information to conform with LSS policy and state and federal regulations. Reviews documentation for compliance

Actively participates in Quality Assurance and Performance Improvement

Initiates and attends resident care plans as needed if no social services coordinator

Addresses and assists in solving residents' and family concerns

Develops a referral network within the community for available resources for residents and families

Manages the Medicaid case load for the respective life plan community or licensed level of living

May conduct closings in AL/ALMC and care center

Develops and changes departmental policies and procedures as needed.

Assists in supporting other departments as needed to benefit the resident and family


Knowledge, Skills, and Abilities:

Bachelor's Degree in Social Work or related field of study required; Master's Degree preferred

Two or more years of related work experience required

Prior experience working in long term care preferred

Social Work License (LCSW or LSW) preferred

Strong supervisory and leadership skills

Strong verbal and written communication skills in regard to fellow employees, resident, families, and outside agencies

General understanding and knowledge of geriatrics

Ability to adjust to daily interruptions and re-arrange schedule due to resident, family and facility needs.

The working conditions involve a well-lighted and ventilated working area that has its own temperature control system (air conditioning and heat); Possible exposure to combative residents; Minimal exposure to chemicals and hazardous waste; Minimal exposure to outside weather conditions; Appropriate ergonomic and personal safety equipment is provided as needed.
