Position:
RN Case Manager

Location:
Boston, MA


Schedule: 8-hour days, Monday to Friday, 7:30 am to 5:00 pm; flexibility to move to 4x10s after the first week of orientation


Requirements:

• Active RN license in Massachusetts
• Minimum of 2 years of experience in case management
• Strong communication and organizational skills
• Ability to work independently and collaboratively within a multidisciplinary team
• Experience in travel nursing preferred

Roles & Responsibilities:

• Conduct comprehensive assessments of patients' healthcare needs
• Develop individualized care plans in collaboration with the interdisciplinary team
• Coordinate and facilitate patient care transitions, including discharge planning and referrals to community resources
• Monitor patient progress and adjust care plans as needed
• Provide education and support to patients and their families regarding their healthcare needs and treatment plans
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care
• Document patient assessments, care plans, and interventions accurately and timely
• Participate in departmental meetings, quality improvement initiatives, and continuing education activities
• Adhere to all regulatory and compliance standards, including HIPAA and CMS guidelines
