The Deep River Center is currently seeking:

Case Manager (m/f/d)

for permanent employment in part-time/full-time.

Your tasks

  • Initial consultation with insured persons and conducting information events in clinics or in their homes
  • Individual case counseling with individual participation planning and corresponding goal setting
  • Planning and coordination of the rehabilitation process as well as support with formal matters
  • Compilation of all necessary data and case documentation
  • Support after successful integration into the job market
  • Establishment and maintenance of networks with service providers, health insurance companies, employment agencies, and institutions
