The Homeless Prevention Council is a nonprofit providing personalized case management services to support self-sufficiency and stability for residents of Lower Cape Cod.
HPC is seeking a Case Manager to work with our dedicated team.Responsibilities
The Case Manager will:
- Assess client's needs to determine their eligibility for various programs
- Develop individualized case plans
- Meet with clients in the office, at their homes or at community outreach locations
- Link individuals and families with communitybased resources
- Assist in budgeting and managing access to various types of assistance
- Ensure confidentiality when handling all aspects of client information
- Work with community partners to provide comprehensive case management services
Qualifications
Pay:
$ $30.00 per hour
Expected hours: 35 per week
