- Cardiology is a clinician dedicated to delivering care for cardiology inpatient and ambulatory patients; the primary area of responsibility will involve clinical performance and documentation of health care delivery for Veterans according to VHA standards.
- Responsibilities include diagnosis, management, treatment, telephone consultation, patient triage, telemedicine, and cardiac device-related problems.
- The Cardiology PA works in close collaboration with Attending Cardiologists, Cardiology Fellows, other Cardiology RNPs, Cath/EP Lab RNs, and support staff.
- Coordinates the referral process and introduction of patients into the Structural Clinic for purposes of evaluation and treatment.
- Guides and directs the continuum of care for structural patients from admission through discharge, including post-discharge care and follow-up as appropriate for the patients.
- Actively participates in research projects that arise from the Structural Clinic as directed by the Lt. Col. Luke Weathers, Jr. VA Medical Center leadership.
- Develops and implements protocols for the care of structural heart patients which are evidence based.
- Develops and implements the educational materials and support needed by the patient and family and assures the patients and families receive appropriate educational counseling to assist them in physical and psychosocial progress toward outcomes.
- Communicates effectively, both verbally and in written format, the progress of the program and identifies areas for opportunity, growth, and improvement to the medical and administrative team on an ongoing basis.
- Coordinate all aspects of the structural CART-CL Registry. Data Registry functions including data collection, data submission, daily quality report issues, outcomes report review, and analysis.
- Assume leadership and facilitation roles with facility physicians, staff, and administrative personnel.
- Receives incoming referrals from physicians and hospitals and responds quickly in order to assure rapid access into the system.
- Communicates back to referring physicians and hospitals the plans for acceptance into the program in an effective manner.
- Educates the patient and family regarding the referral process, steps of evaluation, and keeps patient and family informed throughout the entire process.
- Orders, schedules and manages multiple clinical tests according to protocol to complete assessment process and ensures pre-procedure required testing is complete.
- Participate in the structural clinic as needed. This may include conducting visits with patients.
- Participate in the inpatient structural consult effort as needed. This may include completing a new inpatient consultation or rounding on structural patients in the hospital.
- Pre-procedure Evaluation, Pre-Procedural Care, Immediate Post-Procedural Care, Chronic Clinical Follow-up.
- PA must work closely with colleagues and must serve as a leader in developing processes to improve patient care and satisfaction.
