- Collaborate with healthcare professionals to ensure safe and effective medication use.
- Review and interpret physician orders, monitor drug therapy, and provide drug information to healthcare staff.
- Participate in clinical pharmacy services, including therapeutic drug monitoring, drug utilization evaluations, and pharmacokinetic dosing.
- Contribute to interdisciplinary rounds and collaborate with medical teams to optimize patient outcomes.
- Provide patient education on medication usage and potential side effects.
- Assist in maintaining compliance with regulatory standards.
- Pharm.D (or B.S Pharm. with equivalent relevant experience).
- Current licensure (or eligible to reciprocate) in the State of Delaware.
- Previous hospital pharmacy experience preferred.
