    We are seeking a skilled and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. This is a critical position that requires strong attention to detail and knowledge of patient care, hospice care, and medical coding.

    Responsibilities:

    • Conduct comprehensive assessments of patients' medical records and care plans
    • Ensure accurate and complete documentation of patient information, including medical history, diagnoses, treatments, and medications
    • Collaborate with healthcare professionals to gather necessary information for MDS assessments
    • Analyze patient data to determine appropriate care plans and interventions
    • Monitor and update patient records as needed
    • Stay up-to-date with changes in Medicare regulations and guidelines
    • Provide education and training to staff members on MDS documentation requirements
    • Participate in quality improvement initiatives to enhance patient care

    Qualifications:

    • Valid nursing license (RN)
    • Minimum of 2 years of experience in a healthcare setting, preferably in a nursing home or hospital
    • Bachelor's degree in nursing or related field (preferred)
    • Strong knowledge of Medicare regulations and MDS assessment process
    • Proficiency in medical coding in PCC
    • Excellent attention to detail and organizational skills
    • Ability to work independently and as part of a team
