EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Rikers Island Liaison to work full time for its Community Re-Entry Assistance Network (CRAN) program.
- Friday 9:00AM5:00PM and the office is located in Queens, NY. The role reports to the Program Director.

Primary Purpose of Job:
The Rikers Island based Community Re-entry Assistance Network (CRAN) Community Transitional Case Management (CTCM) Liaison focuses on engaging clients in-person prior to release in order to ensure effective service delivery by CRAN staff.
The CRAN Liaison assists in screening, treatment planning, and case management of eligible substance abusing and/or mentally ill (co-occurring) clients.
Principal Duties & Responsibilities:
- Meet with incarcerated clients with serious mental illness who have been difficult to engage. Explain the CRAN program and otherwise engage each client in a manner that will increase the likelihood of future enrollment and successful engagement with the program.
- Collaborate with discharge planners on Rikers Island and communitybased CRAN Case Managers.
- Complete inperson screenings for incarcerated clients, prior to release.
- Hold groups, small meetings or individual sessions with clients at Rikers facilities.
- Following a comprehensive screening works with supervision to effectuate referrals to mental health, drug abuse, or alcoholism facility. May accompany and/or arranges transportation for clients to the appropriate facility when necessary.
- Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
- Investigate and recommend supportive services for clients.
Knowledge, Skills, & Abilities Required:
- Bachelors degree in Forensic Psychology, Psychology, Criminal Justice or behavioral science with at least oneyear experience with the population required.
- Good communication skills, both oral and written. Ability to work cooperatively with treatment programs, Social Work Department, Department of Corrections and criminal justice systems.
