    Description

    Primary Responsibilities


    The LVN Charge Nurse will help ensure top-quality care to enhance the physical, mental, and emotional well-being of each resident based on assessments and care plans.

    Essential Functions

    • During rounds, observe, assess, document, and report resident condition/changes
    • Ensure implementation of care plans as instructed
    • Administer prescribed medications
    • Conduct rounds with physician and document visits
    • Perform narcotic reconciliation during shifts
    • Recognize significant changes in resident condition and take appropriate action
    • Supervise and monitor aides
    • Complete Incident and Accident Reports as per policy
    • Discharge residents following facility protocol
    • Complete Nursing Assessment for Physician Calls before contacting physician regarding resident condition changes
    • Keep resident's point of contact informed about status updates or changes in condition
    • Fulfill other assignments as directed by the Director of Nurses and/or the Administrator
    Qualifications

    Educational/Training Requirements

    • Must have graduated from an LVN program at an accredited vocational school or college.
    Licensing Requirements

    • Active Licensed Vocational Nurse in good standing in the state of Texas.
    Experience Requirements

    • Preferred experience in a skilled nursing facility.
