    Pontiac Care and Rehabilitation Center -

    We provide skilled nursing, physical therapy, occupational therapy in Oswego and surrounding counties in New York.

    Our patient population is highly diverse constitutes of various ages with emphasis on senior care.

    Director of Nursing Summary of Job Responsibilities:


    We are seeking for a Director of Nursing (DON) who is self-motivated, has previous experience as a nurse supervisor, well organized, and has excellent communication skills.

    The Director of Nursing (DON) will assist us in reaching our mission of growing a quality oriented and an employee friendly organization.

    The right candidate will be responsible for the overall clinical management of our skilled nursing facility.

    Director of Nursing Duties and Responsibilities:

    These duties include, but are not limited to:


    Primary responsibility for team leadership and supervision of clinical staff to ensure that key bench mark indicators are successfully met.


    Perform clinical duties that include intake activities of agency clients, nursing assessment, services plan, care plans, staff assignment, care coordination, client care oversight, periodic on site client visits, staff supervision, and patient reporting.

    Onsite orientation of the service plan to caregivers providing care to a client for the first time.


    Act as a Registered Nurse (RN) advocate and supervisor on behalf of our clients with the physicians, pharmacies, and others that contribute to the care of the client.


    Assist with hiring of appropriate staff and orientation/in-service training serving as a liaison with appropriate management personnel and affiliate departments and committees.

    Implementation of policies and guidelines for home health clinical personnel, providing professional input to related administrative process, and systems.

    Direct and supervise professional and auxiliary personnel rendering patient care services.

    Assist with marketing and communicating the concept of quality client care to the general community.


    Director of Nursing Qualifications:
    Registered Nurse (RN) currently licensed and in good standing in the state of New York.
