Documentation and Eligibility Coordinator


The primary task is to review documented records to ensure the documentation complies with regulations and guidelines from internal & external (IDPH primarily -through the HCWR) quality control practices.

Update/re-certify existing caregiver information to comply with regulation.

This entails verifying that all caregivers who have direct contact to participants are eligible to provide such services given the guidelines and policies that govern us.

Engage in communication with caregivers and/or clients to ensure that both parties are satisfied by services received and provided.

In addition to this you must keep track and implement any changes in the policies as it relates to In Home Service providers.


Responsibilities

  • Filing of initial authorization, reauthorization, terminations and adjustments
  • Control and manages the uploads of all annual documentation requirements (i.e., TB questionnaire, Annual influenza, Hepatitis B,) for existing HMKs
  • Annual eligibility checks and documentations
  • Create an effective system of binder indexing and maintenance
  • Followup with caregivers on expired documents for renewal/update
  • Assist caregivers to troubleshoot problems encountered with Axis Care
  • Required reporting to the supervisors and assistant administrator(s)
  • Maintain "check in and retrieval form" for the File Room so as to keep stock of what goes in and out.
  • Filing of client survey forms.
  • Daily and weekly report on documents created and filed.
  • Complies with the regulations and policies of the company.
  • Creation of Start of Care ( SOC ) binders
  • Come up with a monthly Billing report, Case note report and ADL report for IDOA & IDHS clients.

Requirements:


  • High school diploma or equivalent.
  • Past experience in intake or medical office administration preferred
  • Strong affinity for customer service.
  • Excellent organizational and timemanagement skills.
  • Proficiency in Microsoft Suite

Pay:
From $18.00 per hour
