Education Requirements:
Minimal Bachelor Degree in Human Service Field


Job Goal:
To provide effective outreach and intervention services for identified at-risk students


Performance Responsibilities:


  • Other essential duties and responsibilities may be assigned._
  • Initiate case management of up to 50 truant/potentially truant students to conduct root cause analysis and assess individualized needs.
  • Work directly and collaboratively with a caseload of individual students and their families, school staff, and community service providers to problem solve and implement personalized strategies that address school success and school completion for students in need of additional support to graduate.
  • Determine and document student progress and advocate efforts on a regular basis (i.e., Attendance Success Plan, School Refusal Scale, case notes).
  • Provides and facilitates connections with community, resources for students and families, as needed. Actively seeks out community resources and develops relationships with these resources for future students and/or West40 connection.
  • Attend staff meetings related to students on caseload (e.g., IEP meetings, student support team meetings, suspension reentry meetings, etc.).
  • Regularly check on target students' attendance, grades, and behavior, provide regular feedback to students, communicate regularly with families, and initiate efforts to keep youth engaged in school.
  • Proactively communicates with students, parents/guardians, and/or school personnel in order to support student growth.
  • Responds and intervenes when students are in crisis.
  • Meets regularly with the Truancy Coordinator/Director to evaluate individual cases.
  • Maintains support for students when transitioning successfully out of the truancy program to the next level of support (ALOP, remote school, building contact, etc). Facilitates all necessary contacts to support the transition and remain engaged with students as requested.
  • Leads the development, monitoring, and updating of student progress toward attendance goals. Determines and documents student progress an liaison efforts on a regular basis (eg, attendance success plan, IOEP, case notes, etc).

Terms of Employment:
Twelve-month Year. Salary and work year to be established by the Executive Director


Evaluation:
Performance of this job will be evaluated by the Director of Truancy Outreach & Teen Parent Services in accordance with Board Policy
