- Effectively communicate and implement the Agency's philosophy to staff and the community.
- Collaborate with the hospital team to evaluate patient needs and establish discharge plans.
- Assess patients, their understanding of illnesses, home environments, and support systems.
- Evaluate the patient's ability to comprehend medical procedures and treatments.
- Review referrals, assess care needs, and explain agency services to patients and families.
- Verify insurance coverage and inform patients of financial requirements for home care services.
- Coordinate referrals to other agencies for additional services.
- Arrange necessary medical supplies and equipment for patients.
- Ensure the home health agency is prepared for patient needs post-discharge.
- Provide feedback to the hospital team and participate in staff orientation and performance improvement activities.
- Engage in office duties and responsibilities related to the position.
- Contribute to performance evaluations and goal setting.
- Active New York State RN license.
- Preferably a Bachelor of Science in Nursing.
- 1-2 years of community health experience in a Certified Home Health Agency or Long Term Home Health Care Program.
- Strong problem-solving and prioritization abilities.
- Knowledge of Medicare, Medicaid, and commercial insurance regulations.
- Excellent interpersonal, leadership, and documentation skills.
