Job Summary:

This position will require travel and participation in community events**Work directly with individuals who are experiencing or at risk of homelessness in the East Valley.

Provide support and referrals to service providers to expedite clients' opportunities to gain employment and obtain and retain stable housing.

Assist clients with connections to appropriate resources to address chemical dependency, mental health, and financial concerns as needed to increase community involvement and self-sufficiency, and end homelessness.


Essential Functions:


Identify vulnerable individuals and promote person-centered case planning:
a. Process and track referrals from various internal and external sources, such as community partners and CASS emergency shelter;
b. Outreach and engage clients referred to program;
c. Conduct in-depth intake interviews;
d. Assess client needs and goals to facilitate the creation of an effective, individualized case plan;
e. Establish and maintain a list of community resources;
f. Regularly review case plan with client; provide resource information, referrals, and direction.

Assist clients with skills training, resources, and referrals to increase self-sufficiency and positive outcomes:
a. Identify client's specific needs, their resources, and barriers;
b. Facilitate clients becoming "document ready" for housing and employment;
c. Collaborate with housing providers to expedite housing opportunities;
e. Assess skills and previous vocational experience of clients;
f. Review job listings with clients, providing guidance for pursuing opportunities;

Conduct regular follow-up meetings with clients to provide continued support in achieving case plan goals:
a. Review progress and obtain additional information to support client in progress towards goals;
c. Assist client in obtaining forms, completing paperwork, and contacting referrals;
d. Serve as an advocate for client;
e. Coordinate care with referring agency and other involved providers regularly.

Ensure data integrity for accurate reporting:
a. Create and maintain an updated HMIS enrollment for all clients;
b. Enter complete and accurate notes in HMIS promptly;
c. Compile data and produce reports for analysis and assessment of program effectiveness;
d. Provide "success stories" and other positive client progress information to development team, community stakeholders, and other agencies involved in client's care;
e. Ensure that HMIS files are closed promptly after client's exit.


Other Duties:


  • If needed, provide transportation for housing-related tasks (i.e. apartment tours, accessing emergency shelter);
  • Assist in training newly hired staff;
  • Build and sustain relationships with community partners and stakeholders;
  • Perform other duties as required.

Work Environment:


This job primarily operates in a professional office environment however, occasional off-site venues and outside event areas in various weather conditions are expected.

This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.


Physical Demands:

While performing the duties of this job, the employee is regularly required to talk or hear.

The employee is frequently required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

The employee must frequently lift and/or move objects up to 10 pounds and occasionally lift and/or move objects up to 25 pounds unassisted.

The employee must be able to self-transfer and move throughout the facilities, between buildings, and around campus at multiple sites.

Close visual acuity to prepare and analyze reports and data. Visual acuity and physical ability to operate a motor vehicle.

The individual will need to sit at a desk for extended periods of time to accomplish tasks and must have the manual dexterity in order to use keyboard and mouse.


Position Type and Expected Hours of Work:
This is a full-time position. Days and hours of work are primarily Monday through Friday with time allotted for breaks/lunch. Scheduled hours may vary depending on agency needs. Evening and weekend work may be required as job duties demand. Travel is primarily local during the business day.


Qualifications:


Required:

1. 1-3 years' work experience in social services or relevant field;

  • Strong knowledge of community resources;
  • Possess obtain a State of Arizona Lev
