Description
The Immigrant Health Coordinator will assist in coordination of care for immigrant, undocumented patients at Children's National Hospital who have difficulty accessing health care and other services due to their immigration status.
The coordinator will collaborate across sectors with multiple disciplines to facilitate access to health care, including primary and subspecialty care, medications and other services.
Qualifications
Minimum Education
Bachelor's Degree In a related field (preferred) Or
Associate's degree with 3 years of relevant experience. Or
High School Diploma or GED With 5 years of relevant experience.
Minimum Work Experience
1 year working with individuals or families facing barriers to health care services related to their immigration status. (required)
Required Skills/Knowledge
Spanish Bilingual ability required.
Functional Accountabilities
Delivers Patient/Family Focused Services
- Complete screening interviews of referred patients and families experiencing barriers to care based on immigration status.
- Provide needed assistance, while respecting boundaries and setting appropriate limits.
Coordinates Patient Care Responsibilities with the healthcare team and support staff
- Manage requests from patients and families for hospital or community resources.
- Manage referrals to outside organizations as directed by social work and in coordination with medical team.
Contributes to Effective Multidisciplinary Communication
- Communicate with social work and/or medical team about patient care needs.
- Share verbally and in writing, relevant patient/family information with members of the treatment team.
- Document activities according to Departmental guidelines in electronic medical record.
- Collaborate across disciplines to facilitate access to healthcare services for immigrant families
Optimizes the Use of Available Resources in the coordination of care for immigrant and undocumented patients
- Identify immigration needs and coordinate referrals to Immigration Law resources
- In collaboration with team members, identify resources and facilitate primary and subspecialty care access for undocumented children and youth admitted to Children's National.
- Serve as liaison between the Office of Refugee and Resettlement and Children's National Hospital care providers for patients being reunified in the DMV region.
- Maintain a database of patients in the Immigrant Health Program
- Maintain a database of resources to address needs of immigrant families.
- Assist with development and coordination of a Family Advisory Council for the Immigrant Health Program.

- Meet with T eam Leader for supervision to discuss case material, professional role, and projects to complete.
- Meet with social work or child life partner to review work performed for individual patients and families.
- Participate in Family Service Team meetings and departmental staff meetings.
- Meet departmental requirements for chart recording, statistics, and other documentation and submits documents in a timely manner.
- Ensure that chart documentation is cosigned by licensed Family Services staff member.
- Participate in departmental performance improvement activities.
