Key Responsibilities

  • Conducting physical assessments and monitoring vital signs
  • Performing routine medical screenings, including cognitive and behavioral health assessments through phone and in-person interactions
  • Managing medications and ensuring compliance
  • Administering phlebotomy and handling specimen collection
  • Providing wound care services
  • Maintaining accurate medical records
  • Collaborating effectively with interdisciplinary team members
  • Acting as a primary representative for TANDEM365 in interactions with participants, families, and other healthcare providers

Qualifications and Experience

  • Valid licensure or certification as required
  • Certification as a Medical Assistant
  • Certified Patient Care Technician designation
  • Michigan Licensed Practical Nurse (LPN) license
  • Minimum of 2 years of clinical experience
  • Reliable transportation, valid driver's license, and current auto insurance coverage
  • Access to high-speed internet connectivity
  • Preferred experience in delivering care within community-based settings
