Description
We are looking for a Certified Nurse Practitioner to join our growing Brenham, TX team dedicated to providing patients with quality wound care, chronic medical conditions, and comprehensive medical care in the comfort of their homes, assist living or skilled nursing facility.
Wound Care is a must. Must be willing to travel to surrounding areas (not overnight).Responsibilities Include:
- Performing physical, wound care exams & patient observations in their homes.
- Creating patient care plans & analyzing diagnostic test.
- Diagnosing health issues.
- Ordering, administering, & analyzing diagnostic tests.
- Ordering medications/treatments.
- Consulting w/healthcare professionals and families.
- Detecting changes in patient's health & modifying treatment, as needed.
- Performing Wound Care treatment.
Qualifications Needed:
- Master's degree from accredited nursing program.
- DEA Certification
- Medicare Credentialed
- Wound Care Experience
- APN authorization in the state where employed to practice in family health by the Board of Nurse Examiners.
- RN licensure in the state where employed or a compact party state & boardcertified by either ANCC or AANP.
- Experience providing primary care as a family nurse practitioner.
- Proficiency w/medical instruments & equipment required to perform your work.
- Knowledge of computerbased data mgmt. programs & information systems, as well as medical records & pointofview technology.
- Excellent verbal & written communication skills.
