Possesses clinical knowledge and experience to identify, present and discuss potential patients to providers for program inclusion. Facilitates communication and coordination between all members of the care team to coordinate admissions.Maintains all required documentation in all interactions with our health systems' patients and care team.
Communicates to the appropriate providers about any barriers to the patients' fulfillment of our partner provider's c are p lan.
Associate of Science in nursing .Ability to work on c all, day and evening shifts and rotating holidays .
Bachelor of Science in nursing.
