Job Summary:


Responsible for oversight of all aspects Diabetes Programming, including implementation of project, tracking of data, adherence to protocols, collaboration with community partners, participation in overall activities, quality assurance and improvement, purchases and documentation.


Responsibilities:


  • Collaborates with community partners, Participates in community activities including but not limited to diabetes education.
  • Tracks program expenditures to assure adherence to budgetary limitations and maintains all documentation required for budget justifications.
  • Orders program supplies, including office supplies, outreach materials, metrocards and other items as needed.
  • Develops and submits reports and other documentation as required.
  • Determines the goals, objectives and programming for the nutritional services in the ambulatory setting, including both BronxCare Health System and MLK clinical practices.
  • Responsible for oversight of established policies and for providing overall direction in the continuing operation of nutritional services.
  • Evaluates clinical staff performance through the ongoing provision of supervision and consultation with the Medical Director, Population Health.
  • Provides overall direction of the professional staff within the division to ensure that nutritional services' standards of performance, objectives and fulfillment of responsibilities are met.
  • Reviews and updates nutritional services policies and procedures, staff job descriptions, performance improvement measurement and monitors compliance with standards of care annually.
  • Plans, evaluates and delegates the preparation of monthly and annual statistical, narrative and analytical reports to assess departmental services and hospital needs in relations to the department's mandate and administrative programming.
  • Overall responsibility for the implementation and maintenance of nutritional inservice education and staff development to promote and enhance nutritional counseling skills, knowledge and resources.
  • Conducts monthly staff meetings to apprise staff of hospitalwide performance improvement activities initiatives.
  • Participate in professional association and community based activities as a representative of BronxCare Health System Administration
  • Consistently provides appropriate feedback to employees regarding performance of responsibilities and conducts program evaluation annually.

Qualifications:
Minimum of 2 years experience in program management, team based care and collaboration with community agencies. Familiarity with the health field and issues impacting health care.

Master's Degree -Business Administration/ Social Work (LCSW) or other health related field


Pay:
$126,000.00 per hour
