Position Summary:


Located at Salem Hospital ED, the Emergency Department Nurse Navigator is responsible for accurately and efficiently identifying patients for the MGB Home Hospital program.

This role involves conducting clinical and social eligibility and safety screenings, collaborating with the Home Hospital Admitting provider, Case Manager, and Administrative Coordinator, and communicating with the ED care team, patients, and families to facilitate transfers to MGB Home Hospital.

As the first point of contact for patients, the ED Nurse Navigator must have a strong understanding of the program and be able to accurately present it and address any questions or concerns.


Responsibilities:

  • Attend daily huddles with the Home Hospital team
  • Gather clinical information to identify eligible patients
  • Collaborate with the ED interdisciplinary team and Care Facilitators to identify potential transfers to MGB Home Hospital
  • Work closely with the Home Hospital Admitting Provider to determine eligibility and notify the Case Manager
  • Obtain patient consent for transfer to MGB Home Hospital
  • Communicate with the Administrative Coordinator and Manager regarding patient transfers
  • Maintain realtime knowledge of Home Hospital capacity and capabilities
  • Manage patient flow and placement to support Home Hospital transfers and census goals
  • Utilize analytical and critical thinking to make decisions aligned with organizational objectives
  • Serve as a role model, coach, and facilitator
  • Promote thorough and respectful communication
  • Encourage teamwork
  • Exhibit excellent communication skills that promote patientcentered care, relationship building, and professionalism
