Responsibilities:

Oversee and manage the Case Management department, ensuring efficient and effective operations
Supervise a team of case managers, providing guidance and support in their daily tasks
Conduct regular audits of documentation to ensure compliance with regulatory requirements
Review and analyze data related to case management and utilization review activities, identifying areas for improvement
Develop and implement discharge planning protocols to facilitate smooth and rapid transitions for patients
Provide leadership and guidance in the development and implementation of quality improvement initiatives

Skills:

Strong knowledge of HIPAA regulations and the ability to ensure compliance within the department
Experience in supervising a team of case managers, providing direction and support
Proficient in documentation review, ensuring accuracy and completeness of patient records
Experience in rapid discharge planning
Excellent organizational skills with the ability to manage multiple priorities simultaneously

Position Requirements/Qualifications:

Master's degree in social work or related field.

3 years Hospital Case Management experience required.

Experience working with individuals with drug and alcohol dependencies preferred.

Proficiency in EHR systems.

Pay:
$75, $85,000.00 per year
